  3. Previous Bans from other Servers (If Yes, explain where/when): Once i got banned for no reason and another someone hacked into my account and broke did something bad.

Unfortunately you do not meet the age requirement stated in our ToS. When you turn 13 you are free to re-apply for the whitelist. Until then you may play on the server through the greylist.

Your application is missing the special key which is required. If you read the whitelist guide you will find it. You left your additional info blank as well, which is not something I will accept as well.
