Did some testing for @Kainzo , and removing heros (and the custom items) fixed it.
Seems to work with all stackable items that ARE NOT custom items (Souls, symbols, ect) .
Easiest way I have come to re-creating the bug, which happens alot, is to
1: Drop a stackable item on the ground from a stack.
2: Pick it back up.
3: It is now bugged, and will not restack with the original, until you drop all of that item from your inventory to make them the same.
This is also the case with items that were put into chests. The new items in your inventory will not stack with those in the chests from before.
